ITOKI CORPORATION and the ITOKI Group companies have established the following personal information protection policy and built a personal information protection system. Our company has established a personal information protection program that complies with JIS Q 15001. On February 13, 2009, we were certified by the Japan Information Processing Development Corporation (JIPDEC) as a licensed business authorized to use the "Privacy Mark," which is awarded to businesses that handle personal information appropriately.
Handling of personal information
1. Purpose of the use of personal information
ITOKI CORPORATION (hereinafter referred to as "the Company," "our company," or "we") will use customers' personal information for the following purposes:

In order to handle personal information more strictly, we have established personal information protection regulations based on our personal information protection policy that complies with JIS Q 15001, and operate a personal information protection management system after understanding the external environment. In addition, when actually handling personal information, we take safety management measures from four perspectives: organizational, human, physical, and technical.
(1) Organizational safety management measures
Our company has appointed a person responsible for the handling of personal data, clarified the employees who handle personal data and the scope of personal data that these employees manage, and established a reporting system for notifying the responsible person if any facts or signs of violations of laws or handling regulations are discovered.
(2) Personnel safety measures
We provide regular training to our employees on important points regarding the handling of personal data.
(3) Physical safety management measures
In areas where information assets are handled, we implement measures such as employee's entry and exit management to prevent unauthorized access.
(4) Technical safety management measures
We have implemented mechanisms to protect information systems that handle personal data from unauthorized external access or malicious software. Additionally, we enforce access controls to limit the scope of personnel authorized to handle personal information databases.
3. Provision of personal information to third parties
We will not provide personal information acquired from customers to third parties without the prior consent of the customer, except in the following cases:
(1) Cases based on laws and regulations
(2) When it is necessary to protect a person's life, body, or property, and it is difficult to obtain the consent of the person concerned.
(3) When it is particularly necessary to improve public health or promote the healthy upbringing of children, and it is difficult to obtain the consent of the person concerned.
(4) In cases where it is necessary to cooperate with a national agency, local government, or a person entrusted by them in carrying out the affairs stipulated by law, and with the consent of the person concerned, the When there is a risk of hindering the performance

(2) Request forms for "request for disclosure, etc."
To submit a "request for disclosure, etc.," please click the request form below on the page you are currently viewing and print it out, or request one from the personal information protection desk. Fill in all the required information and enclose a document to verify your identity.
Our company's specified request forms
We will use copies of the following documents as proof of identity:
- Driver's license
- Passport
- Health insurance card

(3) Request for disclosure, etc. by proxy
If the person making the "request for disclosure, etc." is the legal representative of the person in question, a minor, or an adult ward, or an agent authorized by the person in question to make the request, please enclose the following documents in addition to those listed in the previous section.

- One request form specified by our company
- One copy of the person's family register or health insurance card with dependents written on it
- One document to confirm that you are the legal representative of a minor or adult ward, or a power of attorney from the person concerned.
- One copy of the agent's official documents such as driver's license, health insurance card, passport, etc.
(4) Fees for "requests for disclosure" and collection methods
500 yen per application
Please enclose 500 yen worth of postage stamps with your request.
If the fee is insufficient or not enclosed, we will contact you, but if payment is not received within 30 days from the date of our contact, we will treat the request for disclosure as if it had not been made.
(5) Response to "requests for disclosure, etc."
We will respond in writing to the address provided on the applicant's request form.
(6) Purpose of the use of personal information acquired in response to requests for disclosure, etc.
Personal information acquired in response to requests for disclosure, etc. will be handled only within the scope necessary to fulfill the request. Submitted documents will be stored for two years after the response to the request has been completed and will then be properly disposed of.
(7) Reasons for non-disclosure, inability to correct, and inability to suspend the use of personal information subject to disclosure
In the following cases, we will not disclose, correct, or suspend use of the personal information. If we decide not to disclose, correct, or suspend use, we will notify you of our decision along with the reason. Additionally, the designated fee will not be refunded, even if we decide not to disclose, correct, or suspend use.
- If there is a deficiency in the required application documents
- If the identity of the person cannot be confirmed, such as when the address on the invoice, the address on the identity verification document, and the address registered with our company do not match
- When the authority of the proxy cannot be confirmed for an application made by a proxy
- If the subject of the request for disclosure, etc. does not fall under "personal information subject to disclosure"
- When there is a risk of harming the life, body, or other rights and interests of the person or a third party.
- In cases where there is a risk of significantly hindering the proper execution of our business
- In cases where it would result in a violation of other laws and regulations
